Legal Name Verification Request

Effective July 1, 2025, in accordance with the standards for consistent identity management and to reduce the risk of fraud by impersonation, all new and new readmit students at ÃÛ¶¹ÊÓÆµ (ÃÛ¶¹ÊÓÆµ) are required to verify their legal name in our system.

As a result of this procedure, we are required to obtain a copy of the student's official government document to verify that the legal name on the student's record matches the legal name on their document before they can enroll. Therefore, a Legal Name Verification (LNV) registration hold will be added to every new and new readmit student’s account. Please submit your documentation as soon as possible to prevent any delays in registration. New and newly readmited students starting ÃÛ¶¹ÊÓÆµ Fall 2025, have until August 25 to submit their documentation. At that point, we will place holds on all students who have not yet submitted satisfactory documentation.

The policy still allows the student to have a preferred name listed as well, but their legal name must match what is on their government document. USG policy requires that ÃÛ¶¹ÊÓÆµ not recognize requests to use a Preferred First Name that is considered vulgar, offensive, obscene, or otherwise reasonably inappropriate.

Acceptable Documents

  • Driver’s License
  • State-issued I.D.
  • Passport
  • Birth Certificate
  • If your name has changed, you may submit a copy of your marriage certificate, divorce decree, or other government documents that list your name at birth and your current legal name.
  • Military I.D. (copies cannot be made)- must present in person
  • A certified US birth certificate showing the applicant was born in the US or a US territory. Photocopies are not acceptable.
  • Copy of a current US passport.
  • Certificate of Citizenship
  • Certificate of Naturalization
  • Lawful Permanent Resident Card
  • Employment Authorization Card
  • Visa, I-94, I-797 receipt notice, I-20, DS2019, or other immigration document
  • Other Forms Issued by U.S. Citizenship & Immigration Services (USCIS), U.S. Customs & Border Protection (CBP), or U.S. Immigration and Customs Enforcement (ICE)
  • Marriage Certificate
  • Divorce Decree
  • Court Order for Legal Name Change
  • Certificate of Adoption
  • Court Guardianship Documentation
  • FAFSA – We will run a daily process to verify that the name, date of birth, social security number, and citizenship status submitted on your FAFSA matches the name we have on file for you. The FAFSA aid year filed must also align with the academic year you plan to start taking classes at ÃÛ¶¹ÊÓÆµ. If it does not match due to spelling, punctuation, alternative spelling, different last name or first name, etc you will need to submit one of the official government documents above.
